I am recovering from a TFCC tear in my wrist and I'm looking to switch my irons out from steel to graphite to help with the shock to my joints. Right now my irons have a D2 swing weight and the irons I want to go with are D0.

Let's put this in perspective, 2 swing weights equates to about 4 grams, so not a lot. The other thing is swing weight is a crude standard for assessing club head feel. Depending on the balance point of graphite shaft you buy, there might be more weight required in the head, than your current irons, just to hit the D0 target.

Davemac is spot on! The balance point of the shaft can affect the feel in ways the SW will never indicate. Have one built and test it by swinging it and hitting balls. If it doesn't feel EXACTLY how you want it to feel, have the fitter adjust it and continue until you have it perfect. Then build the rest of the set from that.

Built a set of RIP irons, +1/2" and they still were like C8/C9 swingweight. I tried an ultralight grip and that got it to D0. I had to wrap lead tape around the hosel of the club to get it to D2. I like D4, I just gave up.

If you are changing shaft weights...start from the beginning. Don't go into it with a target swingweight, as DM already suggested, start with a single iron at very light swingweight and add head weight till you find the sweet spot, you can then build the others to that spec.
