I stand with my eyes just inside the ball ala Mickelson. I think getting too techy or mechanical can really screw up your putting. My putting turned around when I stopped making conscious efforts to get myself into a certain stance. Now I just get comfortable over the ball, let my arms hang below my shoulders and look down to see the line.

Best advice I recieved in regards to putting is this: setup your putting stance have someone hold another golf ball at your eye level, and have that person drop the golf ball, if the golf ball drops above the ball on the ground, you are standing too close and you will pull your putts. You want the ball drop below the ball on the ground.
