We have done it at my old club, but mind you, it was a weekly club scramble. Not a bigger charity/money/official event scramble. All you do is rotate one extra shot, sometimes we would only give them an extra putt, depending in the group skill level). Contact the committee.

I played in a charity scramble a few years ago and our 4th never showed (Never called either)...right before our tee time I asked the event organizer how he wanted us to handle it.....He said that we could just have one of our group take the extra tee shot....approach shot...putt etc etc....I asked if we should rotate through the team (As someone described above)...he said he didn't care who took the extra shots....I was a 1 'cap at the time...we won by 7 strokes........It is a huge advantage if your best player plays all the extra shots. Rotation is the only fair procedure..

Just like any scramble, there are no written rules. I have played in an event where they announced that the groups with only three get an extra shot. No details about a rotation of who should make the shot or anything like that. I would agree that this would give an advantage to the threesome.
