My Men's club used to use a "card playoff" to break ties for first place in a flight. What they did was to start at hole 15 and compare the cards of the tied players. If both made the same score on 15, it went to 16, and on. If still tied through 18, then it went back to #10 and continued.

Recently played in a low key "friendly" tournament in which the winner of the Senior flight was determined by a card off based on a single hole (the #1 handicap) which I felt was a bit unfair instead of using USGA card off procedures matching back 9, last 6 holes, 3 holes, etc or even better a playoff.

There is no Rule of Golf that dictates how to settle a tie, just guidance from the R&A/USGA. In the first place a full round play off is recommended, then sudden death, and then a "count back" as you describe.

Heck, I had to correct our pro a couple of years ago when we had qualifying for our club championship match play. He said "You guys need to go to #1" and I said "no, YOUR rules that were written down says you start with handicap #1 hole". "Oh, really? ok".
