Get a consistent teeshot, practice irons enough to be semi-accurate. Get the consistency of distance and direction down. When you eliminate those horrible shots or horrible holes you will improve a lot. You should be giving yourself a shot at the green for your second shot on par 4s almost every time, and adjust that for par 3s and 5s. Even if you miss the green, chip it decently close and 2 putt you can get bogeys and thats shooting a 90, giving you 10 strokes of error from that plan. If you manage to hit the green or chip it close, you can get a par and widen your margin even further. But from your score, I'd say you arent on the green in 2 or even 3 most of the time. Thats hugely important. Putting - just be able to 2 putt from anywhere pretty much. Get it close from far away and put it in the hole from a few feet always.
